A supervisor mentions in passing that someone on the team "has been having a hard time with the early shift since her treatments started." No form was filed. Nobody said "ADA." Under the law, the employer’s obligation has very likely already begun.
Many ADA accommodations start this way, which is why the subject causes trouble: the duty is triggered informally, it runs through managers who do not know they are part of a legal process, and it ends in a judgment call.
